After you have an activation code, you can sign up easily on the website.
You must be at least 14 years old to sign up for a MyChart account.

Who can create a MyChart account?
For minors ages 0-13:
A parent/guardian should create their own MyChart account in order to have proxy access provisioned to them to see their child’s information in MyChart. They can request proxy access through MyChart self-signup with their care team. The parent doesn’t have to be a Methodist Le Bonheur Healthcare patient to create an account.
For minors ages 14-17:
Parents’/guardians’ existing MyChart access to the minor’s portal will become limited by default when the child turns 14. Limited access allows parents to see insurance status and immunization records. Minors ages 14-17 can request their own MyChart portal in the clinic or online.
What happens when the minor turns 18?
Existing parent/guardian access will be removed when an adolescent turns 18. The young adult can then grant proxy access if desired.

Will I still be able to access my old portal?
As of June 1, our current patient portal will become read-only until Dec. 31, 2026. You will no longer be able to use the portal and will need to register for MyChart.
Will my information transfer over?
Most of your health care data will be in MyChart on June 1. If you have any questions or cannot see your health care data please contact your clinic location.
Your family history will not transfer over to MyChart or our new electronic health record system. You will need to create a MyChart account and complete the Family History Questionnaire to help your care team provide optimized care.
Will I still be able to message my child’s doctor?
Yes, once you sign up for a new portal, you will be able to send messages to your child’s physician and request test results and medical records through MyChart.
Will I still be able to access my child’s portal?
Yes, you can take care of your children and other family members all from one account.
